Sports physicals are reserved for students in grades 6 - 12. A parent/guardian must be present for the duration of the appointment.
Sports physicals are FREE thanks to University of Delaware's HEALTH for All program!
Registration Required - Once registered, a DIAA form will be emailed to you. Please fill it out and bring it to the appointment.
